PROGRAM GUIDELINES

  1. This program is open to all students 13-18 years old who are residing in Louisiana and Arkansas, United States.

  2. To register, visit us at www.johnsonbehavioralhealthgroup.com and provide your Name, Email Address, and Contact Number.

  3. Once you successfully register, JBHG Team will reach out to you to discuss the next steps of the program.

  4. You will be provided with self-assessment forms you must complete on or before August 12, 2022.

  5. You will receive a follow-up email to further discuss the results of the assessments.
